Infektionen sind häufige Komplikationen der systemischen Therapie hämatologischer und onkologischer Patienten. Sie sind mit hoher Morbidität verbunden, beeinträchtigen die Lebensqualität und können zu signifikanter, Therapie-assoziierter Mortalität führen. Die Arbeitsgemeinschaft Infektionen in der Hämatologie und Onkologie (AGIHO) der DGHO erstellt evidenzbasierte Leitlinien zu Prophylaxe, Diagnostik und Therapie von Infektionen und publiziert sie in Peer-Review-Journals. In Onkopedia sind die Kurzfassungen zu finden.

Das an der Uniklinik Köln koordinierte europäische Impfstudien-Netzwerk VACCELERATE möchte einen Beitrag dazu leisten, dass Studien-Interessierte und Organisatoren von klinischen Studien (z.B. zu COVID-19, Affenpocken, Polio und anderen Atemwegsinfektionen) schneller zueinander finden. Die VACCELERATE Webinare finden ein bis zwei Mal im Monat statt und dauern ca. 1 Stunde. In den Webinaren stellen Sprecher aus verschiedenen Länder die Ergebnisse ihrer Studien vor. Allgemeines, Ziele und Organisation

Ziele und Organisation der AGIHO

In der Behandlung bösartiger Erkrankungen haben begleitende Maßnahmen eine zentrale Bedeutung. Aufgrund der in der Regel bestehenden krankheits- oder therapiebedingten Abwehrschwäche spielen Infektionen eine große Rolle. Die Vorbeugung, Diagnostik und Behandlung derartiger Komplikationen stellt daher eine zentrale Herausforderung in der Hämatologie und Onkologie dar.

Während für Infektionen bei Patienten, die nicht an einer Krebserkrankung leiden, auch in Deutschland Standards in Prophylaxe, Diagnostik und Therapie definiert wurden, existierten derartige Vorgaben für Patienten mit Krebserkrankungen bis Mitte der 1990er Jahre nicht in gleicher Weise.

Die Arbeitsgemeinschaft Infektionen in der Hämatologie und Onkologie, eine Fachgruppe der Deutschen Gesellschaft für Hämatologie und Medizinische Onkologie e.V. (DGHO), hat sich 1996 konstituiert mit dem Ziel, diesem Mangel abzuhelfen und Konzepte zur Kontrolle therapieassoziierter infektiöser Komplikationen zu entwickeln.

Struktur der Arbeitsgemeinschaft

Unsere 1996 gegründete Arbeitsgemeinschaft setzt sich aktuell aus mehr als 200 Ärztinnen und Ärzten aus Deutschland und Österreich zusammen, die ein besonderes Interesse an der Erforschung und Behandlung von Infektionen bei krebskranken Menschen haben.

Der Vorstand besteht aus Herrn Prof. Dr. Oliver Cornely, Köln (Vorsitzender) und Frau Prof. Dr. Christina Rieger, München (Stellvertretende Vorsitzende).


Ziele und Tätigkeiten der Arbeitsgemeinschaft

Ein wesentliches Ziel der AGIHO ist es, Leitlinien zur Prophylaxe, Diagnostik und Therapie von Infektionen bei Patienten mit hämatologischen und onkologischen Erkrankungen zu erstellen. Diesem Ziel kommt die Arbeitsgemeinschaft durch Bildung von themenspezifischen Arbeitsgruppen nach, die diese Leitlinien regelmäßig aktualisieren und auf den zwei- bis dreimal jährlich stattfindenden Treffen der Arbeitsgemeinschaft zur Diskussion stellen. Dreizehn evidenzbasierte Leitlinien der AGIHO sind bisher in internationalen Fachzeitschriften wie Annals of Hematology, Annals of Oncology, European Journal of Cancer und Haematologica veröffentlicht worden und sind auf dieser Webseite z.T. verfügbar ("Leitlinien"). Ein zweites Ziel der AGIHO ist die Unterstützung klinischer Studien zur Epidemiologie, Prophylaxe, Diagnostik und Therapie von Infektionen bei hämatologischen und onkologischen Patienten. Auch hier sind bereits international publizierte Studienergebnisse verfügbar, so in Annals of Oncology und im Journal of Antimicrobial Chemotherapy. Aktuelle Studieninitiativen finden Sie ebenfalls auf dieser Webseite unter "Studien".

Zum Zwecke der klinischen Fortbildung im Bereich der Epidemiologie, Prophylaxe, Diagnostik und Therapie infektiöser Komplikationen bei Patienten mit hämatologischen oder onkologischen Erkrankungen führt die AGIHO jährlich einen zweitägigen Ausbildungskurs mit Beteiligung zahlreicher Experten auf diesen Themengebieten durch. Auf den Jahrestagungen der Deutschen, Österreichischen und Schweizerischen Gesellschaften für Hämatologie und Onkologie richtet die AGIHO jährlich ein Symposium "Update Infektionen in der Hämatologie und Onkologie" aus. Anlässlich des alle zwei Jahre stattfindenden internationalen Symposiums "Acute Leukemias" in München richtet die AGIHO englischsprachige interdisziplinäre Symposien zu organbezogenen Infektionen, klinischen Differentialdiagnosen und Therapieempfehlungen aus.

EPICOVIDEHA - Epidemiology of COVID-19 infection in patients with hematological malignancies: A European Haematology Association Survey
EPICOVIDEHA aims to better understand the epidemiology of COVID-19 in patients with hematological malignancies (including hematopoietic stem cell transplant recipients) in the different European countries. The results obtained will allow us to better know the prevalence of this complication in the different categories of patients with hematological malignancies. In order to attain the objectives previously described we will develop a multicentre, international, observational, retrospective and prospective study of consecutive cases of COVID-19 among hematological malignancies. There will be a clinical follow-up of the patients included in this study to observe the survival rate. Data collected form this study will be evaluated with a descriptive analysis.

Inclusion criteria
- Age >18 years old
- History of / active/ diagnosed hematological malignancies at any stage/status
- Positive SARS-CoV-2 by RT-PCR

Exclusion criteria
- Hematological diseases, other than hematological malignancies
- Not tested positive for SARS-CoV-2
- Patients “off therapy” for more than 5 years for hematological malignancy

OncoReVir erfasst alle Fälle von viralen Atemwegsinfektionen bei Krebspatienten seit Oktober 2017 fortlaufend. Primärer Endpunkt ist die Rate der Pneumonien, sekundäre Endpunkte sind u.a. Intensivpflichtigkeit, Überleben und Identifikation von Risikofaktoren für ein schlechteres Outcome. Die Dokumentation erfolgt in schlanken eCRF. Die Studienleitkommission besteht aus Prof. Dr. Marie von Lilienfeld-Toal, Jena, Dr. Nicola Giesen, Heidelberg und Dr. Daniel Teschner, Mainz. Das Projektmanagement ist im Universitätsklinikum Jena angesiedelt. Aktuell nehmen 9 Zentren der AGIHO teil.

FungiScope ist ein globales Register zur web-basierten Sammlung demografischer und klinischer Daten zu Patienten mit seltenen invasiven Pilzinfektionen sowie zugehöriger klinischer Pilz-Isolate für zentrale Laboruntersuchungen. Mit Hilfe dieser umfangreichen Datensammlung sollen neue Erkenntnisse über die Epidemiologie, Diagnostik und Therapiemuster gewonnen werden, die in Zukunft zur verbesserten Prävention und Therapien beitragen können.
Auf www.fungiquest.net erhält man eine anonymisierte Zusammenfassung klinisch ähnlicher Fälle, die in FungiScope erfasst werden. Daten zu PatientInnen mit gleichem Krankheitserreger, gleicher Grunderkrankung, klinischer Manifestation, Therapie und ähnlichem Outcome können mit Hilfe einer Suchmaske gezielt abgefragt werden und sollen ÄrztInnen als Unterstützung im klinischen Alltag dienen.

Young AGIHO – gegründet 2023

Die Young AGIHO soll die Interessen junger Ärztinnen und Ärzte sowie Studierender vertreten, die sich am Beginn Ihrer Karriere oder in der Weiterbildung befinden und hohes Interesse an Infektiologie in der Hämatologie/Onkologie haben.

Ziele

  • Mitwirken von Assistenzärztinnen und Assistenzärzten sowie Studierenden an der aktiven Arbeit der AGIHO, bspw. bei der Leitlinienarbeit oder bei gemeinsamen klinischen Studien als Prüfärztinnen und Prüfärzte, Dokumentare oder bei der Erstellung von Studienprotokollen
  • Aufwertung klinisch-wissenschaftlicher Arbeit in der Aus- und Weiterbildung
  • Verbesserung von Karrierechancen für junge Ärztinnen und Ärzte
  • Qualitätssicherung der Weiterbildung in der Hämatologie/Onkologie und Infektiologie sowie die Motivation junger Kolleginnen und Kollegen eine Karriere in diesem Bereich zu verfolgen
  • Einbindung von Assistenzärztinnen und Assistenzärzten nicht-akademischer Krankenhäuser in die Arbeit der AGIHO

Themen

  • Partizipation von Weiterbildungsassistent*innen und Studierenden in die Arbeit wissenschaftlicher Gremien
  • Förderung der klinisch-wissenschaftlichen Weiterbildung
  • Vereinbarkeit von Karriere und Familie
  • Aufklärung über die verschiedenen Beschäftigungsmöglichkeiten für junge Ärztinnen und Ärzten sowie Studierenden im Bereich der Infektiologie und/oder Hämatologie/Onkologie
  • Networking durch gemeinsame Veranstaltungen und regelmäßigen Austausch
  • Förderung studentischen Interesses an unserem Fachbereich

Perspektiven

  • Die Entwicklung zu einer Weiterbildung in der Infektiologie und/oder Hämatologie/Onkologie, in der Elemente eines Clinician Scientist Programms enthalten sind bzw. die Teilnahme an einem solchen Programm durch aktive Mitarbeit vorbereitet werden kann
  • Aktives Netzwerk an jungen Ärztinnen und Ärzten sowie Studierenden in unserem Fachbereich zur Nachwuchsförderung und Vertretung eigener Interessen
  • Beteiligt euch und werdet teil eines dynamischen, jungen Karrierenetzwerks

Young AGIHO – founded 2023

The Young AGIHO (Working Group Infections of the German Society of Haematology and Medical Oncology) represents interests of young physicians and medical students who are still in training or at the beginning of their careers and who provide a high interest in infections in haematology and oncology.

Aims

  • Include young physicians in training for infectious diseases or haematology/oncology in the active work of AGIHO and clinical decision making, i.e., working on clinical guidelines or as trial physicians in clinical studies of the network, also drafting clinical study protocols
  • Improvement of the acknowledgement of clinical and scientific work during medical school and residency
  • Improvement of career chances and opportunities for young physicians during training
  • Inclusion of physicians from non-academic medical institutions and hospitals in the active work of AGIHO
  • Encourage young physicians in training to pursue a career in infectious diseases in haematology/oncology and ensure quality of training / education in the area
